UPA Athletics Participation Policy & Requirements

• Complete both the Physical Exam and Health History Form

• Upload all required documents to SportsNet before tryouts (no paper copies accepted)

• Full SportsNet registration + medical clearance required for participation

• No conditioning, tryouts, practice, or games without completed registration and medical clearance

How to Register

1. Click “Create an Account” at the bottom of the page

2. Enter all required information and submit

3. Make sure you have 26-27 selected upon returning to the login page.

4. Log in using your email and password

5. Follow the on-screen instructions and upload your forms

6. Sign electronically at the end to complete registration

7. A clearance email will be sent once your student is approved
